Scheduled appointments are encouraged as they allow for more time with the practitioner.
However, we recognize the importance of timely treatment of acute illness.
East Hampton Family Medicine accommodates walk-in care when necessary.


Covid-19

Masks are required for all patients and visitors over two years of age. Patients are asked to keep their masks on throughout their visit, unless directed by a provider to remove it.

Scheduling your first appointment

  1. Call our office at 631-324-9200 to schedule your appointment. If required, please make sure your insurance company recognizes that Dr. George Dempsey is designated as your primary care provider before you arrive for your appointment.

  2. Bring your insurance card, photo ID, and if possible, your records from previous medical providers.

  3. Bring in your medications, including over the counter medicines and supplements.

  4. Please arrive 15 minutes prior to your appointment in order to complete your medical history for your first appointment.

If you are having symptoms that you feel are life threatening, immediately call 911 or go to the nearest Emergency Room.

Medical Records Release

In order to facilitate your visit, please fill the following form out and fax it to any office we are requesting records from. If you need assistance please feel free to come in to the office so we can assist. Obtaining outside records is of utmost priority to us as your Primary Care Physician, so do not hesitate to request our help. Authorization to Obtain Information


Payments / Co-payments

Please have your co-payment ready when you arrive at East Hampton Family Medicine. It is due each time you see a provider, and is based on your health insurance contract.
